$499 / plan year
- 3 periodontal maintenance visits
- Comprehensive exam, periodic exam & emergency exam
- 2 oral cancer screenings
- 2 fluoride applications
- Unlimited diagnostic X-rays
- 20% off periodontal & laser treatments
- 15% off eligible dental treatment*

Special Member Discounts
- Orthodontics — $300 off
- Implants — $200 off
- Dentures — $150 off
These discounts apply in place of the standard 15% treatment discount.
